Who qualifies for 9/11 head and neck cancer compensation?
Anyone diagnosed with head and neck cancer following presence in the covered 9/11 areas, at the Pentagon, or in Shanksville, Pennsylvania during the covered periods may qualify for VCF compensation. This includes area workers, residents, students, teachers, first responders, cleanup and recovery workers, volunteers, and surviving family members of those who died from a 9/11-related head and neck cancer.
Free medical care runs through the WTC Health Program, which covers Manhattan south of Houston Street and any block in Brooklyn within 1.5 miles of the former World Trade Center site. Compensation runs through the VCF, which covers Manhattan south of Canal Street plus routes of debris removal.
If you were between Canal and Houston Street, you may still qualify for both. Daily life in Lower Manhattan rarely stopped at Canal Street, and presence can be established with records or sworn affidavits.
Which head and neck cancers does the WTC Health Program cover?
The WTC Health Program's list of covered conditions names head and neck cancers by body site, in four separate entries. Together they reach the mouth, throat, nose, sinuses, middle ear, and voice box:
- Mouth and throat: the lip, tongue, salivary glands, floor of the mouth, gums, tonsils, oropharynx (the middle of the throat, including the base of the tongue and soft palate), hypopharynx (the lowest part of the throat), and other parts of the oral cavity and pharynx.
- Nasopharynx: the upper part of the throat, behind the nose.
- Nose, sinuses, and middle ear: the nose, nasal cavity, middle ear, and accessory sinuses (the small air spaces in the bones around the nose).
- Larynx: the voice box, which holds the vocal cords.
Most of these cancers are squamous cell carcinomas. That means they begin in the thin, flat cells lining the moist inner surfaces of the mouth, nose, and throat.
Does the WTC Health Program treat thyroid cancer as a head and neck cancer?
No. The WTC Health Program lists thyroid cancer as its own covered condition, with a shorter minimum latency of 2.5 years. Our 9/11 thyroid cancer page explains how those claims work.
The same is true of cancers of the esophagus, the eye and orbit, and the skin of the face and neck. Each has its own entry on the WTC Health Program's list, and each is still a covered condition. The National Cancer Institute does not usually classify them as head and neck cancers either. The full list of covered conditions shows where each one falls.
How did 9/11 exposure cause head and neck cancer?
Head and neck cancers are covered conditions in the WTC Health Program, but each member's cancer must still be certified individually.
To certify a head and neck cancer, the WTC Health Program must determine that the member's 9/11 exposure is substantially likely to be a significant factor in aggravating, contributing to, or causing it. That determination is made case by case, alongside the WTC Health Program's exposure and minimum latency requirements.
What are the symptoms of 9/11-related head and neck cancer?
Common signs include a lump in the neck, a sore in the mouth or throat that does not heal, a sore throat that does not go away, trouble swallowing, and hoarseness or a change in the voice. Other, less serious conditions can cause the same symptoms.
Signs vary with where the cancer starts:
- Mouth: a white or red patch on the gums, tongue, or lining of the mouth; swelling of the jaw that makes dentures fit poorly; or unusual bleeding or pain.
- Throat: pain when swallowing, neck or throat pain that does not go away, pain or ringing in the ears, or trouble hearing.
- Voice box: trouble breathing or speaking, pain when swallowing, or ear pain.
- Nose and sinuses: blocked sinuses that do not clear, sinus infections that do not respond to antibiotics, nosebleeds, frequent headaches, swelling around the eyes, or pain in the upper teeth.
- Salivary glands: swelling under the chin or around the jaw, numbness or weakness in the muscles of the face, or lasting pain in the face, chin, or neck.

Head and neck cancers fall in the WTC Health Program's solid cancer category, which carries a minimum latency of 4 years. At least 4 years must pass between the initial 9/11 exposure and the initial diagnosis. This latency requirement is set by the WTC Health Program and has no exceptions.
The condition must be certified as 9/11-related by the WTC Health Program, or through the VCF Private Physician Process if an exception applies.
Sometimes cancer is found in a lymph node of the neck and no primary site can be identified. The National Cancer Institute calls this metastatic squamous neck cancer with unknown (occult) primary. The WTC Health Program's Administrative Manual provides for certifying cancers of unknown primary origin, using the date the metastatic disease was diagnosed.
No head and neck site appears on the WTC Health Program's list of in-situ (non-invasive) cancers that are automatically ineligible. Non-invasive forms at these sites are evaluated case by case.

Treatment for a certified head and neck cancer runs through the WTC Health Program at no cost to the member. Compensation is a separate claim with the VCF.
Awards can also include economic loss, such as lost earnings, qualified out-of-pocket medical expenses, and wrongful death compensation in fatal cases.
